PAtient Portal Information

TIPS FOR GETTING STARTED
• Choose an e-mail address to receive
(Each e-mail address will be the User Name for that child)

• Think of an easy password to remember that is at least 8 digits and alpha numeric with a special character (You can have the same password for all the children)

• When you are ready to web enable the children, download the Patient Portal Consent Form and fill out the E-mail Registry. You can bring it to your next appointment or fax it to us at (847) 296-6435.

PATIENT PORTAL STEPS:

1. Sign the Patient Portal Consent Form and fill out the E-mail Registry.
   Then send to Sub Peds
   Sub Peds will enter email address and web enable patient
2. (Optional) Sub Peds prints users information and temporary password and gives to patient
3. Patient will receive an email to each of the email addresses set up for each child
4. Log in using User Name (email address) and temporary password.
5. Use Date Of Birth to validate account
6. Set up New Password and Security Question
7. You are finished – keep email address and passwords somewhere safe for future use.

TROUBLESHOOTING

Call (847) 296-5205 if you have trouble logging in.

Complete the following steps to get set up in the system:

1. Obtain a copy of your child/children’s Medical History from your previous pediatrician.
* Under 5yrs old requires COMPLETE Medical History
(Vaccine Record, Growth Charts, Probelm List, Specialist Consult Notes & Complete Doctors Notes.)

* Over 5yrs old requires last 2yrs of Medical History
(Vaccine Record, Growth Charts, Probelm List, Specialist Consult Notes & last 2yrs of Doctors Notes.)

2. Send COMPLETE Medical History to Suburban Pediatrics, Ltd via fax (847) 296-6435 or by mail or email.

3. Call Suburban Pediatrics, Ltd at (847) 296-5205 to add your child/children to our system & make an appointment. (Please allow 24 - 48 hrs for the COMPLETE Medical History to be received and reviewed before calling to be added.)

4. To allow time to complete the necessary paperwork, please arrive 15mins prior to your appointment time. You can also download the Info & Consent Forms, fill out and bring to your appointment along with your insurance card.
